Use up to four office visits at no additional charge during each 12-month membership period. Visits may include a new patient evaluation, routine follow-up, chronic disease management, medication management and prescription refills, preventive care, annual physicals, or evaluation of acute illnesses.
Your annual physical counts as one of your four included visits.
After your four included visits, additional eligible office visits cost $50 each, compared with the regular $75 follow-up rate. There is no limit on additional visits, subject to appointment availability.
Receive 10% off eligible cash-pay laboratory testing purchased through Nuvia Health. Laboratory testing is billed separately.
Same-day appointments are available based on provider availability. Our experienced nurse practitioners provide individualized internal medicine and preventive care, including diabetes, blood pressure, cholesterol and thyroid management, acute illness visits, medication management, and chronic disease care.

We require an initial six-month membership commitment. After that, your membership continues on a month-to-month basis.
Yes! Your four included visits are available throughout your 12-month membership period. You do not have to wait a certain number of months between appointments.
You can continue scheduling appointments for just $50 per additional office visit.
No. Unused visits expire at the end of each 12-month membership period and do not carry forward.
Yes. An annual physical may be completed as one of your four included office visits.
Laboratory testing is billed separately. Members receive 10% off eligible cash-pay laboratory testing purchased through Nuvia Health.
Prescription evaluation and management are included as part of your office visits when medically appropriate. The cost of prescription medications is not included.
Unused visits have no cash value and are forfeited when your membership ends.
Our membership program is available to uninsured, self-pay patients who are not using health insurance to pay for services covered by the membership.
Yes. After completing your initial six-month commitment, you may cancel your membership with 30 days’ notice.
Membership payments already made are nonrefundable, except as required by applicable law.
A new membership period begins when you reenroll, subject to the membership terms in effect at that time.
